Is level of management control adequate?
Is there documentation of site rules and are they distributed?
Is the effectiveness of the plan is regularly reviewed?
Are your supervisors, drivers and others, including contractors and visiting drivers, aware of the site rules?

Is the level of supervision sufficient while ensuring standards are followed?

Are loading/unloading activities carried out on ground which is properly levelled?
Are parking brakes always used on trailers and tractive units to avoid unwanted movement?
Are the vehicles brakes stabilized to avoid unsafe movements during loading/unloading operations?
Is there a proper system to avoid trucks driving away while they are still being loaded/unloaded?
Are lorry drivers and others kept in a safe place away from the vehicle while loading/unloading is in action?
Is there a safe area marked visible to drivers when loading ?
Is there a limit for the number of people in the load area?
Is there an appropriate lifting equipment for loading/unloading?

Is there a properly designed one-way system used on vehicle routes?

Where sites are not stable, are the tipping operations safe?

Are drivers aware of when tailgates should be removed?
Prior to commencing tipping operations, do drivers check even distribution of load across the vehicle?
Are the drivers sufficiently experienced to anticipate loads sticking?
Do drivers always ensure that the body is completely empty, and drive only a few metres forward to ensure the load is clear?
